"Our surveys say the transformation is going well, but delivery hasn't changed."
The Sense layer finds where adoption is stuck by comparing two kinds of evidence:
The barrier point is the earliest stage where the evidence is weakest, or where the two sources disagree. A team that reports high confidence while its work signals stay flat is often showing a gap between belief and practice. That gap is the conversation to have.
The signals follow the order of the change itself. Clarity about the why comes first, and resilience shows up last, so when an Embed signal drops, check the Engage signals before reaching for a new game.
Healthy
Struggling
Review the pulse and signals together in the Sprint Retrospective. Name one barrier point, then hand it to the team to design the play.
Drill-down pressure: someone asks who is behind a weak signal. The answer is always the team, by design.
A team's pulse is strong across all six checkpoints. Its Delivery Confidence signal is flat, and late scope changes keep appearing. The barrier point is Pressure, in the Equip stage: the team knows the practice but doesn't yet do it under pressure.
Leaders see the same team-level picture after the team does: which stage is the barrier point, and whether it is moving. They never see individual answers.
